Melanoma is the most malignant skin tumor and one of the most aggressive. It arises from the melanocytes of the basal layer of the epidermis. It is mainly found in the skin but can also appear in mucous membranes such as the eye, meninges and intestine.

The main causes of melanoma are exposure to ultraviolet radiation, phenotypic characteristics: Fitzpatrick phototypes I and II, a large number of melanocytic or dysplastic nevi, large congenital nevi, personal or family history of melanoma, hereditary mutation in the CDK4 and CDKN2A genes, malignant lentigines, congenital nevi, melanocytic nevi, chronic irritation of nevi, immunosuppression, while changes in female hormones during pregnancy or taking birth control pills are implicated.
Melanoma is characterized by a series of clinical features known as
ABCDE rule. It is very important in early diagnosis which will determine in
to a large extent and the prognosis of the disease:
- Asymmetry: asymmetry
- Border: irregular border
- Colour: color (recent color change or variegation)
- Diameter: diameter > 6 mm
- Elevation: elevation or palpable nodule
Melanoma can metastasize to lymph nodes and also hematogenously to distant organs. For this reason, early diagnosis by a dermatologist through clinical examination, dermoscopy and ultimately biopsy is very important. The next step after diagnosis is the staging of the melanoma. This is determined by measuring its thickness, the depth to which it has reached the skin and whether there are metastases to adjacent lymph nodes or other organs of the body.
TREATMENT
Depending on the stage, the type of treatment will be determined. Surgery is the main method of treating melanoma. Radical resection of the primary tumor is performed and, depending on the stage and the sentinel lymph node, the patient undergoes lymph node dissection of the affected area. In advanced forms of melanoma, chemotherapy, immunotherapy (interferon, ipilimumab) and radiotherapy to a lesser extent are applied.
